You'll receive a text message from Little Jacob asking you to meet him. Once you arrive at the destination and get in the car, he'll tell you that he's followed some of Pegorino's men to the building just on your right, and that he believes they'll lead them to Pegorino and Dimitri. He also informs you of Pegorino and Dimitri's new found friendship.
The cutscene begins, and once the mobsters exit the building, they are aware of Niko and Jacob immediately. However, due to a meeting with the 'Peg', they'd rather not get in trouble for missing that, and try to shake off the company instead.
You now have control of the vehicle. The mobsters will start firing at you for the first few seconds, but other than that they do not pose much of a threat and all you need to do is concentrate on keeping track of the car. There shouldn't be any complications doing so since both yours and their car are relatively slow (in comparison to say, a Turismo), but they do pull a few tricks and you must effectively use your brakes to avoid this.
After you've followed them to a certain area, a cutscene begins where Niko and Jacob arrive at the Old Casino where Jimmy and Dimitri are hiding. Niko spectacularly skids his car across and slides across the hood to take cover from incoming fire. Niko and Jacob have a chat, and Niko convinces Jacob to not fight, and instead get him transport out of there. Once Jacob's gone, you'll resume play.
From here you're in cover behind the car. This bit is fairly easy providing you take your time and keep in cover. Use the auto-aim to eliminate the three guys on the rooftop, and use your auto-aim to lock on to the targets that are hiding behind the cars.
Tip: If you have a rocket launcher, blow some of the cars up and save some time.
Advance forward slowly and take cover on the wooden planks and kill the remaining enemies out here. Once you've cleared outside, it's now time to go inside.
When you reach the door you'll receive a text message from Jacob telling you to keep your eyes on the sky.
This section is similar to outside; take cover just to the side of the door and slowly eliminate the enemies inside using auto-aim. There's a guy on the stairs on the left you'll need to take out first, and then the guys hiding behind cover straight ahead of you. They'll be most likely to hit you out of everyone, so it's important that you take care of them as soon as you can. Once the room is clear, grab the health pack from the portacabin and go through the archway.
A cutscene begins where Dimitri executes Pegorino.
Run up the stairs on to the roof and pick off the four or five waiting enemies here. No cover is required, a submachine gun should do the job splendidly. Climb on to the roof on your right and move to the other side to trigger another cutscene.
Dimitri and a goon have made off in a helicopter, where Niko manages to grab on to. The goon kicks Niko off, into the water, conveniently next to a boat.
Get into the boat and immediately make chase after Dimitri's helicopter. He'll start to fire rockets at you, so be careful, but it's absolutely crucial you keep close to the chopper. After some rocket dodging for a good few minutes, Little Jacob arrives ahead in an Annihilator. Keep moving forward and Jacob'll begin to land; a little blip appears underneath the helicopter. You need to drive your boat underneath Jacob's helicopter before it's too late. If you make it in time, Niko'll leap from the boat into the helicopter. Tap A repeatedly to get into the chopper.
You now are commandeering the helicopter. Keep on Dimitri's tail and watch out for his rockets. Fire a few shots in if you can, although it's unconfirmed whether it actually affects his helicopter or not. Jacob will eventually shoot Dimitri's Maverick down, but you also get hit too, and another cutscene begins showing both helicopters crashing on to Happiness Island.
Dimitri makes a run for it, and the island is full of his goons. There's a lot of them scattered around. You can either ignore them and watch your health deteriorate or you can be patient and kill them all. Either way, once you reach Dimitri, you need to pump him full of lead. He takes quite a beating so be careful of where you're positioned otherwise you may be vulnerable to his attacks as well.
Kill him and the final cutscene triggers. Niko walks up to the dying Dimitri and says a few words. Jacob encourages Niko to leave him.
Congratulations, you've completed the storyline.

If The Price Is Right - Mission Walkthrough
So you've opted to work with Dimitri once more.
You have to meet up with Phil Bell for this mission, as he'll be assisting you with the deal. Go to the green marker and get in the truck that's waiting for you. You'll need to drive Phil to the boatyard, where a drug deal is going down; Dimitri will be handing over the drugs at a seperate location and you're there to collect the payment.
Once you arrive at the gate, a Russian mobster will confirm that you're there to collect the cash and will send you in. Once you park up, a cutscene starts. Niko receives a call from Dimitri who has, no surprise, screwed him over again. He's decided to wipe out all of the Russian buyers and keep the drugs all for himself, and advises Niko that the Russians still have the money, and if he wants it, he's got to go and fetch it before they figure out what's going on.
Follow Phil on to the roof. He advises that you must strike now before it's too late. Follow him in to the warehouse and kill all the gangsters you can see. There's plenty on the ground floor by the trucks, but there are a few on the walkways where you and Phil are situated, so be careful here. You have all the time in the world, however, so make sure you utilise the cover feature and advance slowly through rather than going all guns blazing.
Follow Phil through the balcony and steadily make your way along the walkways. Ensure you keep Phil covered, don't let him get too roughed up.
Anyway, turns out the guy with the money is safely tucked up in an office. You'll have to stay close to Phil and follow him until he reaches the office. He'll require your help, and once you reach him a cutscene begins showing Phil kicking the door down. Once you help Phil open the door, he'll make a run for it with the cash. Follow him down the stairs, making sure to kill the last few Russians here, and go outside. He's made off in a truck, so find the waiting pickup and follow the (rather slow) truck with your money in it. No point wasting your ammo until the truck gets on to the main road, though.
Once you have killed the driver of the truck, pick up the money and take Phil back to his place in Tudor.

Out Of Commission - Mission Walkthrough
After Little Jacob has sent you a text, head to the gun store and buy some weapons. If you already have some, give him a call. He'll tell you him and Roman have spotted one of the guys responsible, and they're waiting in a car in Alderney.
Grab a vehicle and make your way to meet Roman and Jacob. Once you arrive, get into the car with them. You'll see a short cutscene where the guy Jacob has been watching will attempt to escape in a car. You need to chase him all of the way back to Pegorino. Follow him across the bridge and listen to the conversation between Jacob, Roman and Niko.
Eventually you'll arrive at the mafia building. Niko tells Jacob and Roman to keep back and work out an exit plan. You're going to be going in to take out Pegorino.
One the cutscene's over, you'll have about ten Mafia guys shooting at you. Stay behind the cover of the car and pop up to take them out one at a time.
Once you can no longer hit anyone from that range, sprint forward and take cover behind the sand bags. Finish off the Mafia guys out here, then make your way towards the house. As you approach the door, Jacob will tell you he's got an escape plan and that you should keep your eyes on the sky. Obviously he's got a helicopter.
Anyway, head to the doorway and take cover against it. There'll be a bunch of guys at the back of the room, and some up and to the left on the platform. Take them all out, then move inside. You'll find a health pack stuck to the wall on the building to the left, so grab that.
Move through the room and go through the door to the back left. After the cutscene, Jimmy Pegorino will run to the roof. Chase him up the stairs. Once you get outside, there'll be two gyards waiting for you further down the roof, so take them out and grab their ammo if you need it. Head down the ladders and kill the three enemies who come charging towards you. Once they're dead, you could head back up the ladder and into the stair well. There was a health pack on the other side of the steps which might come in useful. Jump over the gap to climb up and get it.
Head back outside and down the ladder, then make your way along the side of the building. Take cover behind the objects as you go, and take out any enemies who are doing the same. As you move around the corner a car will appear and attempt to block the path. I was able to free aim and shoot the driver and passengers straight in the head.
Chase Jimmy down to the docks and jump on the bike. He will take off, and you need to chase him along the coastline. Make sure you get to this part quickly or he'll get away.
Speed after him and eventually you'll come to a part where the Mafia have the road to the right blocked off. Instead, head to the left and go over the wooden ramp, marked with a yellow light. Speed off that ramp and you'll grab hold of the helicoper. You now need to mash the run button to climb in. Once you're on board, you'll need to chase after Pegorino and use the helicopters miniguns to take him out.
Keep shooting at him, but regardless, the helicopter will be hit by a couple of RPG's. Luckily Niko manages to land it at the docks to Liberty Island before it crashes and blows up. He tells Jacob and Roman to wait there while he finishes this.
Chase after Pegorino and kill the two guards he will have by the metal railings, then chase after him. He'll hide behind a tree near the hot dog guy, so make your way over to him and fire a few bullets into him.
Watch the final cutscene where Pegorino bleeds a lot before Niko finishes him off. Jacob and Roman then tell you that you did it. It's somewhat fitting that the music that plays at the end of the game is the same music that plays in the first trailer.
